Replacement of seaman's certificate requires a copy of ID card; 4 copies of small certificate; Copy of service manual; Copy of seaman's certificate; Copy of competency certificate; The driver also needs a copy of GMDSS certificate, 3 2-inch photos and a physical examination form.
