First of all, Han Yangchun, deputy general manager of safety, reported the situation of preventing coal mine fires in Zaozhuang. After 63 people were rescued, 28 people were still trapped.
In view of the fire prevention situation in Zaozhuang Coal Mine, in order to learn from the accident and make inferences to ensure the company's safety in production, Han Yangchun, deputy general manager of safety, arranged as follows:
(1) The mine manager of each mine will take the lead in immediately organizing a mine fire safety inspection, and relevant professionals and personnel of the Ministry of Security will participate in the inspection of each mine to ensure that the inspection is meticulous and no dead ends are left. If the hidden dangers found can be rectified immediately, they should be rectified immediately; If it cannot be rectified immediately, the Ministry of Security shall urge it to rectify within a time limit. After the inspectors and re-inspectors sign, the rectification situation will be filed.
(2) The fire safety inspection organized by each mine should be targeted, mainly to check the reliability of underground fire control facilities, the safety of power supply facilities, the implementation of fire prevention measures, and the effective control of underground fire sources, so as to enhance the fire prevention awareness of underground construction personnel and ensure mine fire safety.

Minutes of fire accident meeting Fan 20XX On June 24-25, the Safety and Environmental Protection Department, Engineering Department and Chemical Industry Department of China Petrochemical Co., Ltd. jointly organized a meeting of Maoming Petrochemical Company? 6.3? On-site seminar on fire accidents. Representatives and invited experts from Maoming Branch, China Petrochemical Engineering Construction Company, Luoyang Engineering Company and Qingdao Safety Engineering Research Institute attended the meeting.
At the meeting, representatives and experts from all units listened to the reports of Maoming Branch and Qingdao Safety Engineering Research Institute on the preliminary investigation of the accident, and made an on-site inspection of the accident. The meeting was divided into six groups: electrical, torch, automatic control, equipment, technology and comprehensive management to discuss the accident. The opinions of the meeting are summarized as follows:
I. Causes and nature of the accident
This is a liability accident caused by the quality problem of molecular sealing equipment of D torch contractor, which led to the shutdown of flare system and eventually led to the fire of the cracking gas pipeline of HB- 102 cracking furnace.
Second, after the accident
20XX June 3 18: 32, 1 10KV, the overhead line of the northern line of the external power grid was struck by lightning, resulting in the short circuit fault between A and the ground, resulting in the power loss of No.1 power supply of Ethylene North Transformer 1. After a short-term power failure (within 2 seconds), the automatic power switching device was automatically turned on, and the power supply was successfully restored. However, due to short-term power failure, 1# polypropylene, 2# polypropylene, 1# high-pressure polyethylene, high-density polyethylene, 2# butadiene, ethylene glycol, 2# hydrogenation, air compressor C- 103A/C, thermoelectric furnace 1, 3# generator and 2# methane cracking hydrogen compressor CB. Due to the defects in the manufacturing quality of D torch molecular seal, the bell jar in the molecular seal fell off and blocked the torch barrel, which led to poor torch discharge and pressure suppression. 18: 46, the flare system pressure reached 0.22MPa, which led to the interlock shutdown of No.2 cracking intermediate pressure cylinder CB30 1 dry gas seal with high exhaust pressure. About 250t/h cracked gas was discharged to flare system, and the flare system pressure rose to 0.59 1Mpa, which exceeded the design pressure (0.35Mpa) of the cracked gas pipeline at the outlet of the cracking furnace.
Third, the main problems of accident exposure
1, power supply and distribution
1) 1 10KV line lightning protection measures are not perfect.
2) There is a contradiction between the economic operation of generator and the safety of power supply and distribution system.
2. Torch discharge system
1) From the site, there are serious problems in the welding quality between molecular seals, some welds are incomplete, and the weld structure does not meet the specification requirements. The manufacturer failed to provide complete quality control documents.
2) The design documents provided by No.711Research Institute of China Shipbuilding Industry Corporation, the contractor of D torch, are incomplete, and the calculation of molecular seal pressure drop and structural design of D torch are not perfect, which is far from the design requirements of equipment intrinsic safety.
3) The connection between the fixed rib plate and the bell jar in the molecular seal is unreasonable, which makes the connection a weak link.
3.2# Boiler Outlet Cracking Gas Pipeline Expansion Joint
The design switching time of the large valve of the cracked gas pipeline at the outlet of 2# boiler is 195 seconds. After a period of trial production, the switching time is as short as tens of minutes and as long as 1-2 days. In order to solve the problem of long switching time, an expansion joint was installed on the cracking gas pipeline, but the setting process of the expansion joint did not meet the relevant management regulations.
4. Integrated management.
1) The existing emergency plan does not solve the problem of excessive pressure in flare system, nor does it have a disposal plan when the pressure in flare system is excessive.
2) The external information release after the accident is insufficient.
3) At the scene of the accident, especially in bad weather, the existing communication tools can't communicate effectively.
Four, should draw lessons from the accident and put forward rectification suggestions.
1. Maoming Branch should contact Guangdong Power Supply Company and carefully analyze the ownership of the power supply company. High frequency line protection? Ensure the removal time of 1 10KV incoming line fault.
2. It is suggested to install lightning arrester and other lightning protection measures on 1 10KV overhead line to improve the lightning protection level of1/0kV line. Ethylene 1 10KV North has become the key substation of ethylene, so the installation of fault recorder should be considered.
3. Measures such as fast switching, anti-sloshing contactor, optimized restart, and adjustment of under-voltage protection setting value of high-voltage motor should be comprehensively evaluated, necessary power system analysis and calculation and dynamic simulation experiments should be carried out, and targeted rectification should be carried out to ensure the safety of power system and prevent the accident from expanding after failure.
4. Pay attention to the management of flare system equipment, such as molecular seal, optimize the design of bell jar in molecular seal, improve the strength and rigidity, improve the welding connection between bell jar and rib plate, increase the legs of plug, and strengthen the monitoring of its manufacturing quality. It is suggested to check the molecular seals of the same type in China Petrochemical Company.
5. Pay attention to the process management in flare system, including the de-coagulation of water-sealed tanks and molecular seals, set up a high-pressure alarm for torch gas, and list the molecular seals of the torch in the required inspection items during maintenance.
6. Check the pre-tightening force and stress of the existing cracked gas pipeline during installation, confirm whether it is consistent with the design situation, and recalculate and adjust the existing deviation.
7. Check the actual strength and rigidity of the cracking gas valve currently in use and its matching with the pipeline, organize relevant experts to demonstrate the difficulties in opening and closing the existing valve and put forward rectification suggestions.
8. It is suggested to add a pressure transmitter with the range of 0-0.6Mpa at the top of TB-220 quench water tower of 2# cracking unit.
9. Make emergency plan for failure in flare system and conduct drills.
10, strictly manage the change, and comprehensively and systematically analyze the risks brought by the change.
1 1. In view of the adverse effects caused by some media hype after the accident, it is suggested to seriously sum up the experience and lessons in accident information release and establish an effective accident information press release mechanism and procedure.
12, report the accident in time in strict accordance with the accident reporting regulations of the group company.
