Visa application form 157W

When filling in the 157W form, please fill in each item truthfully. Please fill in "N/A" (not applicable) even if some information is irrelevant to you.

Please be sure to fill in the complete supplementary review form.

Highest academic certificate and report card

In order to pass the evaluation smoothly, please provide the certificate and transcript (English version) reflecting your highest education. If the school can't provide English certificates and transcripts, you can take the transcripts and certificates to a translation company for translation or to a notary office for translation and notarization.

List of family members

Please fill in the complete family member form (sample form 54).

registration certificate

Please provide proof that you are currently studying (English version). The certificate issued by the school should include the name, date of birth, position, signature of the person in charge, date of issuance and other information.

household registration

Please prepare a Chinese and English account book. As the household registration unit generally cannot provide the English version of the household registration certificate, I suggest you send it to the notary office for translation and notarization.

Economic guarantee and financial proof

Please prepare relevant materials to prove your financial strength and guarantee ability.

Resume or study plan

Resume should be concise and focused. You can write it as an autobiography, introducing your experience since graduation, the reasons why you chose to study in Australia and your future study plans. If there are honors or special circumstances that need to be explained, they can also be included in your resume.

Physical examination report and X-ray chest film

Please have a physical examination in advance and prepare relevant reports and X-ray chest films.

Australian school admission notice

When applying for a visa, you need to provide an admission notice from an Australian school. Make sure to receive the admission notice from the school and keep it properly.
