Standardized training base for residents of Municipal Health and Family Planning Commission:
? 20 15 year unified examination for standardized training of residents in the whole province has ended. There were 4,534 residents who took the exam, of whom 3,307 passed. The list of qualified personnel in the examination (attachment 1) is now published, and the relevant matters for handling the certificate of qualification are notified as follows:
? I. Requirements for Issuing Certificate of Conformity
? According to the relevant provisions of the state and our province, those who pass the first-stage examination of standardized training for residents can obtain the first-stage certificate of standardized training for residents uniformly printed and distributed by the Provincial Health and Family Planning Commission, and those who pass the second-stage examination can obtain the standardized training certificate for residents.
? Two. Procedures for applying for a certificate of conformity
? (1) Summary of the list of qualified personnel. According to the unified examination results of standardized training for residents in 20 15, the training base hospital carefully reviewed the qualified personnel and daily training assessment at this stage, and determined the list of qualified personnel. Fill in the Registration Form for Persons Applying for the First Stage Qualification Certificate of Standardized Resident Training in 20 15 (Annex 2) and the Registration Form for Persons Applying for Standardized Resident Training in 20 15 (Annex 3) (hereinafter referred to as the Qualification Certificate Registration Form) and report it to the Municipal Health and Family Planning Commission for review. Fill in the Summary Table of Qualified Residents in the First Stage of Standardized Resident Training in 20 15 (Annex 4) and the Summary Table of Qualified Residents in 20 15 (Annex 5) (hereinafter referred to as "Summary Table"), and submit the Registration Form of Qualified Certificate and the Summary Table electronically before August 2/kloc-0. The medical institutions affiliated to the Economic Commission shall be submitted to the provincial residential training center for review.
? (2) Obtaining a certificate of conformity. On August 26 -27, the medical institutions affiliated to the municipalities and economic commissions (departments) went to the provincial residential training center to receive the certificate, and filled in the certificate code in the corresponding column of the certificate registration form. The training base hospital carefully checks all the information in the certificate. After it is confirmed to be correct, paste the recent two-inch color bareheaded photo of the candidate himself, and affix the official seal of the training base hospital in the corresponding position.
? (3) Go through the printing inspection procedures. On September 6-7, the personnel of the medical units affiliated to the municipalities and the Economic Commission in China went to the provincial residential training center to go through the printing inspection procedures with the certificate, certificate registration form and number summary table. ? Three. Related matters
? (a) other units entrusted to participate in the unified examination of qualified personnel, by receiving the training of the base hospital unified certification. When filling in the Registration Form of Qualified Certificate, the training base hospital shall indicate the name of the work unit entrusted by the training personnel in the remarks column, fill in the Information Form of Resident Trainers Entrusted by Other Units (Annex 6), and submit it to the provincial resident training center when going through the printing inspection procedures.
? (2) The certificate fee is 15 yuan.
